This list is built from at least four lists off the Internet plus
quite a few extra I've added. Remember to contact both the place you're
moving from and the place you're moving to where these are under different
companies/authorities. I.e. moving from London to Swansea will mean a new
doctor, dentist, water board, electricity company, school etc. so require
two letters, while changing your address on your driving licence is only
the one letter. Most of these organisations will require the change in
writing.
While this list has been written by a British person, I've tried
to make sure it applied equally to the US and the rest of the world ...

to get any repeat prescriptions etc. transferred)

You will need your driving licence if you want to hire a moving van, so
don't send it off until after you've finished the move!

Key return service, if your keychain/fob has a tag saying "if found, drop
this in any mailbox" you'll want your keys back!

Other Useful Addresses/Phone Numbers to Get
Aside from the organisations/people listed above, the following addresses/phone
numbers may be useful, you should also consider leaving or passing on a
list of the major numbers above and below to whoever is getting your old
property next! Don't leave them your credit card numbers unless you're
passing your old house on to your children <grin!>

Late night shops (particularly petrol/gas, food, chemist/pharmacy)

Emergency numbers (glazier, gas, electric, water, roofer etc.) Many credit
card companies have a special hotline to assist with these
